In exchange for letting my child participate in the SandStorms Ice Hockey Club activities, including practices, games, tournaments, camps and other events:

1. I agree that my child and I have talked about the possible risks, dangers and hazards associated with the sport of ice hockey. I understand that, even with protective equipment, participating in the activities listed above, whether on or off the ice, puts my child at risk of a possible severe or fatal injury to himself/herself or others. These risks include, but are not limited to, injuries from:

• strenuous cardiovascular workouts;
• exerting and stretching various muscle groups;
• rapid movements and quick turns and stops on the ice;
• collision with the rink boards, hockey nets, and ice;
• being struck by hockey sticks and pucks;
• physical contact with other participants;
• variations in the ice surface; and
• participating with older, bigger, faster and/or stronger players.

2. I acknowledge that while playing ice hockey, my child could suffer injuries to his or her eyes, teeth, face, head, and other parts of his or her body, such as bruises, cuts, scrapes, breaks, dislocations, and spinal cord injuries that may render him or her permanently paralyzed.

3. On behalf or my child, I fully accept and assume all the risks, dangers and hazards associated with playing ice hockey and participating in Sandstorms activities and I release the

SandStorms and all SandStorms volunteers and staff from any liability or claim for any injury or loss that may occur during any SandStorms activity.

4. My child has no known medical condition that would prohibit him or her from playing ice hockey.

5. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my family, including child(ren), and I may be exposed to or infected by COVID-19 while on site at the venue and that such exposure or infection may result in personal injury, illness, permanent disability, and fatality.

6. If I am not present during a SandStorms activity where my child is injured, I authorize treatment deemed necessary and I will reimburse the SandStorms for any expense incurred in relation to such treatment.

7. When my child participates in SandStorms activities, I will observe the principles of fair play as follows and I will:

• encourage my child to play hockey by the rules and regulations as set by the SandStorms and in the spirit of the game and to resolve conflict without resorting to hostility or violence;
• remember that children learn by example and so I will applaud good plays and performances by both my child’s team and their opponents;

• never ridicule or yell at my child, or any other child participating in SandStorms activities, for making a mistake or losing a game;
• show respect to all coaches and officials and neither berate them nor question their judgement in public; and
• respect and show appreciation for the volunteers who give their time to the SandStorms organization.
